Course and Faculty Member Evaluation Form

This form asks for your feedback and suggestions about your experience taking this course.  Even if you only fill out the check-boxes on the first page, or if you are generally satisfied with the course, your input is helpful.

Student input is very important to Birthingway, and while it is in no sense a requirement or necessity, we beseech you to fill out this form carefully and thoughtfully.  All student evaluations are read by the Faculty Coordinator and the instructor in question, and many are read by other members of the administrative staff. Your suggestions and your reactions to the course are helpful in planning for the next time this course is taught. All suggestions are considered carefully.

Students are welcome to submit course evaluation feedback in any form, just mark it “course evaluation” and hand it in to the Front Office.

Administrative staff, including the President, maintain an open door policy to receive student comments orally, but will usually request the student to give these comments in writing as well in order to insure their accuracy. Please be aware that, while a personal conversation may create a greater sense of immediacy in giving your input, writing your comments is a more efficient way of getting them “on the radar” and into the hands of people who can utilize them to make changes.

Students sometimes question whether giving their feedback does any good, since it may sometimes appear that “nothing ever changes”.  Please keep in mind that the improvement of course content and teaching skills is a long-term developmental process both for the instructor and for the College.  There are many instances of student input resulting in changes: different textbooks have been adopted, teacher training programs have been instituted, new teachers have been hired, course learning objectives have been updated, and other actions have been taken that may not appear as dramatic changes.
